About Us
Faith Walk Support is a registered National Disability Insurance Scheme (NDIS) provider based in Melbourne, Victoria. We deliver person-centred support services to participants in line with their approved NDIS plans, with a focus on dignity, consistency and respect.
Services We Provide
We offer Assistance with Daily Living and related disability support services as described on our website and agreed in your service agreement. All services are delivered in line with the NDIS Practice Standards, the NDIS Code of Conduct and applicable Australian law.
Service Agreements
Before supports commence, we provide a written service agreement that sets out the supports to be delivered, schedules, fees, responsibilities and cancellation terms. Both parties must review, agree to and sign the service agreement before services begin.
Fees and Payment
Fees are charged in accordance with the current NDIS Pricing Arrangements and Price Limits. Payment is managed through your NDIS plan funds (self-managed, plan-managed or NDIA-managed) or as otherwise agreed in your service agreement. Invoices are issued in line with the agreed billing schedule.
Cancellations and Rescheduling
We ask for at least 48 hours' notice for cancellations or rescheduling so we can re-allocate our support workers. Short-notice cancellations may be charged in line with current NDIS cancellation rules and your signed service agreement.
Privacy and Confidentiality
We collect, store and use personal information in accordance with the Privacy Act 1988 (Cth) and NDIS requirements. Your information is treated as confidential and is only shared with authorised parties where necessary to deliver your supports, or where disclosure is required by law.
Participant Rights and Responsibilities
Participants have the right to safe, respectful and culturally appropriate support, to be involved in decisions about their care, and to raise concerns without fear of retribution. Participants are asked to provide a safe environment for our workers and to communicate openly about changing needs.
Complaints and Feedback
We welcome feedback and take all complaints seriously. Please contact us directly so we can resolve concerns quickly. If you are not satisfied with our response, you may escalate to the NDIS Quality and Safeguards Commission on 1800 035 544.
Limitation of Liability
To the maximum extent permitted by law, our liability for any service is limited to the resupply of that service or the refund of fees paid for it. We are not liable for indirect, incidental or consequential losses arising out of or in connection with the supports provided.

We may update these terms from time to time. The current version will always be published on this page with the latest update date shown below. Continued engagement of our services after changes are published constitutes acceptance of the updated terms.
